Housing RepresenativeAs you prepare for move-in, please remember to complete the following steps.

Priority deadline for all requirments to be COMPLETED is May 31, 2024. 

Final deadline for all requirments is June 30, 2023 at 5pm. 

No students will be permitted, under any circumstance, to move into the residence hall if these steps are not complete. 


To be eligible to live in the Residence Hall, a student must be enrolled as a CSU student. Priority is given to full-time students taking (12) semester credit hours for undergraduate and graduate students taking (9) credit hours. The department reserves the right to deny housing to students not registered for the corresponding semester. Registration must be completed before moving into housing. Please contact the campus registar office to complete registration. 

Financial Aid Questions

Complete FAFSA for the 2024 – 2025 school year. Each housing applicant must complete a Financial Aid Housing Eligibility Form. Students must have filled out his or her FAFSA before applying for on-campus housing. For questions contact:  

Application Fee

A $100 non-refundable application fee will be applied to the student's account upon applying for housing and must be paid before the final housing deadline by June 30, 2024. Criminal Background Check

All new and returning housing students must complete a criminal background check. There are two parts of the criminal background check. Part I is the Campus Safety Questionnaire in the housing application. Part II is to complete your background check by visiting, fees will apply. Use package code: cg78

Medical/ Immunization Questions

All students need to be medically compliant to live in Housing. All Medical Records need to be submitted and cleared with the Wellness Center. If any of the below is missing, for returning you will be in violation of the Housing Agreement which could jeopordize your housing and for new incoming students this will prohibit you from obtaining housing. Physical Exam History

    • Diphtheria and Tetanus (TD), must have been within the last 10 years.
    • Tuberculin (TB), skin test with results within the last year.
    • Complete Physical, completed within the last year.
    • 2 doses of Measles, Mumps, Rubella (MMR), or a test verifying immunity. 

Submit all medical records to the Wellness and Center either via email at, and/or fax at 773-995-2953 (please confirm receipt of records if faxing/emailing). 

Contact the Chicago State University Wellness and Health Center by phone at 773-995-2010 for questions, assistance in obtaining immunizations, or laboratory (serologic) testing. The Wellness Center is able to conduct the medical requirements at the Wellness Center on campus for FREE for students. 

*International students are able to arrive on campus and complete the TB test to recieive results within 3-4 days. It is the students' responsbility to secure housing if they are tested positive for TB. Please reach out to the Wellness Center for further questions. 

Be sure to make two copies of records. Send one copy to the Wellness/Health Center and maintain one copy for your record. Please do not send any medical records to housing.
