| To be eligible to live in the Residence Hall, a student must be enrolled as a CSU
                                 student. Priority is given to full-time students taking (12) semester credit hours
                                 for undergraduate and graduate students taking (9) credit hours. The department reserves
                                 the right to deny housing to students not registered for the corresponding semester.
                                 Registration must be completed before moving into housing. Please contact the campus
                                 registar office to complete registration.  Financial Aid Questions Complete FAFSA for the 2025 – 2026 school year. Each housing applicant must complete
                                 a Financial Aid Housing Eligibility Form. Students must have filled out his or her

                           | Application Fee A $100 non-refundable application fee will be applied to the student's account upon
                                 applying for housing and must be paid before the final housing deadline by June 30,
                                 2025. This fee needs to be paid yearly for EVERY resident. Please do not mail the

                           | Medical/ Immunization Questions All students need to be medically compliant to live in Housing. All Medical Records
                                 need to be submitted and cleared with the Wellness Center. If any of the below is
                                 missing, for returning you will be in violation of the Housing Agreement which could
                                 jeopordize your housing and for new incoming students this will prohibit you from
                                 obtaining housing. 
 Physical Exam History
 
                                 
                                 Diphtheria and Tetanus (TD), must have been within the last 10 years.Tuberculin (TB), skin test with results within the last year.Complete Physical, completed within the last year.2 doses of Measles, Mumps, Rubella (MMR), or a test verifying immunity.  Submit all medical records to the Wellness and Center either via email at wellness@csu.edu, and/or fax at 773-995-2953 (please confirm receipt of records if faxing/emailing).  Contact the Chicago State University Wellness and Health Center by phone at 773-995-2010 for questions, assistance in obtaining immunizations, or laboratory (serologic) testing.
                                 The Wellness Center is able to conduct the medical requirements at the Wellness Center
                                    on campus for FREE for students.  *International students are able to arrive on campus and complete the TB test to recieive
                                    results within 3-4 days. It is the students' responsbility to secure housing if they
                                    are tested positive for TB. Please reach out to the Wellness Center for further questions.  Be sure to make two copies of records. Send one copy to the Wellness/Health Center
