Health Insurance is Mandatory.

Chicago State University provides a mandatory Injury and Sickness Insurance Plan underwritten by United Healthcare Insurance Company. All full-time students (undergraduates enrolled in 12 hours or more, graduates enrolled in 9 hours or more, and doctoral students enrolled in 6 hours or more) are automatically charged for the plan. All full-time international students are required to purchase the insurance plan and are eligible for the plan. The fee is applied to the accounts of students enrolled in all on-campus courses or a combination of off -campus (Extension) and on-campus courses.

Waiver/Enrollment Process

Students are required to complete an online form to waive out of or enroll into the mandatory CSU insurance plan every fall and spring semester. Failure to submit an online waiver or enrollment form by the deadline will obligate students to pay the full-time (FT) Student Insurance Premium coverage indicated on your tuition bill. A confirmation email from the UHCSR Student Center will be required for any waiver dispute.

To Waive: Students who have health insurance coverage that is at least comparable to the CSU plan are able to waive the mandatory health insurance plan by going online during the specified waiver/enrollment period and completing the waiver form provided there. To submit a waiver form, go to https://studentcenter.uhcsr.com.

To Enroll: Students who do not have insurance or simply want to keep the plan must go online to complete and submit an enrollment form. Submitting an enrollment form will help expedite activation of your enrollment should you need access to benefits prior to when the university submits its final list of students who are to be insured. To submit an enrollment form, go to https://studentcenter.uhcsr.com.

Students Who Take No Action: Emails are sent to every student who is charged for the mandatory insurance plan informing them that they have been charged, and encouraging students to submit an enrollment or waiver form. Students who do not submit an online waiver by the announced deadline will be required to buy the mandatory insurance plan.

Waiver/Enrollment Period and Deadlines: The waiver/enrollment periods for the Spring 2013 regular 16-week and the special 12-week sessions are as follows: (a) Regular 16-Week Session = November 19, 2012 thru January 21, 2013; and (b) 12-Week Session = January 18, 2013 thru February 18, 2013. Waiver/enrollment periods and deadlines are also published every semester in the Academic Calendar, on the Office of Financial Aid web page, on the Student Health Insurance web page, and under Cougar Connect Student Resources.

Period of Coverage: Students enrolled in the mandatory insurance plan for the Spring 2013 sessions will be covered as follows: (a) Regular 16-Week Session = January 14, 2013 through August 18, 2013; and (b) Special 12-Week Session = February 11, 2013 thru May 17, 2013. Part-Time Students, Spouses and Dependents Eligible for Coverage
Part-time students enrolled in 6-11 credit hours may participate in the university plan on a voluntary basis. Part-time and full-time students enrolled in the Regular 16-Week Session who are themselves insured in the plan may insure their eligible dependents, spouses and domestic partners on a voluntary basis. The enrollment period, deadline, and period of coverage are the same as those for CSU students.
