Registration Information

BY MAIL OR FAX: Contact the Department of Extension Services at (773) 995-2214 for a mail in/fax registration form. Mail the extension registration form to the Department of Extension Services, Chicago State University, 9501 S. King Drive, JDC 201, Chicago, Illinois 60628. You can also fax your registration form to (773) 995-2941. The deadline for registering by mail or fax is August 30, 2011.

ON CAMPUS: Students registering exclusively for off-campus courses can register in the extension services department, JDC 201, from 9:00am to 4:30pm If enrolling in a combination of on-campus and off-campus courses, students must register via CSU X-Press.

IN CLASS: Unless noted otherwise in the individual class information, you may register at the first session of a class, if the course is still open. You are strongly urged to register prior to the first class since some courses fill quickly.

ON THE WEB: Students who are formally admitted into a degree, certification, or certificate program must obtain a current registration advising PIN (RAP) from their advisor during the advisement session. An advising PIN is not required for at-large students. However, course prerequisites must be met in order to register. At-large students may begin registering online during the periods of May 4 through August 3, August 8 through August 17 and August 20 through 21.

FEES: Tuition is based on the status of the student, not the status of the class. Tuition is subject to change.

TUITION PAYMENT OPTIONS: Call the CSU Cashier’s Office directly at (773) 995-2029 to make a payment using a credit card (Discover, MasterCard or VISA.) You may also submit check or money order payments by mail to the Department of Extension Services, 9501 S. King Drive, JDC 201, Chicago, IL 60628. Write the correct course number and section number on the check or money order. Please enclose a telephone number where you can be reached, if required, on the check or money order.

INSTALLMENT PLAN: Only students registered for six hours or more are eligible for the installment plan. There is an additional service fee based on the installment plan selected. Students choosing the installment plan option must utilize the tuition management installment plan, as presented in the Fee and Payment Information section.

CLASS SIZE: The university reserves the right to limit the size of classes and to cancel due to insufficient enrollment.

CLASS MEETING: Normally, classes meet once a week for 16 weeks. Sessions consist of 50 minutes of instruction each week for each credit hour, (i.e., a three credit hour course meets 150 minutes per week). Courses meeting for fewer than sixteen weeks compensate with more frequent and/or longer class meeting each week, as detailed in the individual class listing.

Fall 2011 Extension courses begin September 6.

PREREQUISITES: Course prerequisites are stated in the Chicago State University Catalog or on the web site: www.csu.edu/catalogs/.

GRADUATE ADMISSION APPLICATION (NON-DEGRE STUDY):

Students enrolling in graduate courses for the first time and who are not currently admitted to a graduate degree program are required to complete a graduate-at-large application. A $25 application fee must be submitted with the at-large application. Additionally, proof of a bachelor’s degree must be submitted with the application, (i.e., copy of a transcript illustrating an awarded degree, copy of diploma, or state teaching certificate). Graduate at-large students who have not enrolled in a course at CSU within two years or more, but left in good standing, are required to apply for readmission by completing a readmit form. There is a $25 fee for readmission. Students admitted to a program who have not attended CSU within the last two years must obtain a departmental advisor’s signature. Submit your extension registration form, at-large application or readmit form, proof of degree, tuition fee and a $25 readmission application fee to your instructor or an extension services department representative on the first day of the class, or submit to the extension services department, JDC 201, if enrolling only in extension courses.
