Withdrawal Process

Students can initiate an official complete withdrawal from CSU in the Office of the Registrar located in the Cook Administration Building Room 128. Students should complete the following steps to complete the process:

  • Student should obtain the Change of Schedule/Withdrawal, WP-WF Withdrawal, or Special Session Registration form in the Office of the Registrar.
  • Student must fully complete the form including their CSU email address and CSU identification number as well as indicate the semester in which they wish to withdraw.
  • Student must enter the discipline/subject, course reference number(s), credit hour(s), and section numbers for all registered classes.
  • Student must go to the Office of Student Financial Aid and meet with a financial aid advisor to determine potential financial liability upon withdrawing and have the form signed and dated.
  • Student must meet with an account representative in the Office of the Bursar to determine financial liability to CSU and have the form signed and dated.
  • Student will meet with either his/her Academic Advisor or the Chair of the Department (if the advisor is not available), to determine academic standing and discuss a re-instatement plan to the University, prior to having the form signed and dated.
  • Student must sign and date the form and return it to the Office of the Registrar to complete the withdrawal process.
