1.	Use this sheet to provide written comments on specific task force recommendations. Only written comments will be considered although live hearings are scheduled on these dates.
	Tuesday, April 26, 2005 ED 102 12:00 p.m. – 2:00 p.m. Thursday, April 28, 2005 ED 102 12:00 p.m. – 2:00 p.m. Friday, April 29, 2005 ED 102 5:00 p.m. – 7:00 p.m.
	Verbal statements made at the hearings should be accompanied by a completed feedback form
2.	All recommendations are preliminary and must be approved by the President and Board of Trustees of Chicago State University. Use one form for each recommendation.
3.	After April 18, 2005, task force recommendations will be available on the strategic planning Web site (http:www.csu.edu/strategicplanningresources/index.htm) and in the Office of Academic Support (SUB 158).
4.	Send the completed feedback form to Delores Lipscomb, Chair, Strategic Planning Steering Committee (SUB 158) no later than May 16, 2005.
