CSU - Housing and Resident Life Application for Student Housing

This is the official Housing and Resident Life student's application for the CSU 2008-2009 academic year. Please submit your medical records directly to the CSU Wellness/Health Center, Cook Administration Building, Room 131, phone (773) 995-2010, fax (773) 995-2953 to obtain a green medical compliance card. Please submit receipts for your housing application and/or housing fee with your application page and green medical compliance card after you have made your payment directly to CSU Cashiers Office. You may also mail a check or money order to the Department of Housing and Residence Life, if you are out of state with your application.
