Guidelines for Submission of an Academic Petition

Please fill the form out in its entirety. Your completed petition must:

  1. be a clear, concise statement of your request and why it should be considered.
  2. have attached documentation when appropriate.
  3. have the signatures of the faculty person, chair and or       dean when appropriate.

Requests to drop, add or withdraw after posted deadlines must also have the appropriate form attached with the requested signature(s).

Please note:  If your petition is illegible or  grammatically incorrect, no action will be taken, and your petition will be returned to you intact.

Petitions will be acted upon within two (2) weeks.  During peak periods, i.e., end of term or registration, more time may apply.
