Professional Staff Report Form for NCA Self-Study

As you are aware, CSU is undergoing a self-study in preparation for the North Central Association (NCA) evaluation visit schedule for April 2003. A part of the self-study requires that various reports relating to the professional staff be prepared. In order to amass this information, each full or part-time professional staff member is asked to complete a professional staff report. A copy of the professional staff report should be forward to Delores Lipscomb, NCA Coordinator, SUB 158, by February 1, 2002.

The report should follow the format presented in the sample report attached to this document. You are asked to head the first page as presented in the sample provided. Information included in the heading should include: your name, your title and/or rank and the department in which you rank is held, and your date of appointment at CSU.

You are asked to provide information related to each of the items presented below. All reports should be typed. Respond to each item in order. Use as many pages as you need in order to present the information requested. Type all item numbers and the headings as presented in capital letters. If you have nothing to report for a section, type N.A. after the item heading.
