Strengths
Actions Taken (Improvements) Since The Last Visit -- Indicate the improvements and the evidence to support them. Provide copies of actual documents to support the improvements
Weaknesses All weaknesses should be included in the departmental strategic plan and on Form 207E of the Long-Range Plan. List also weaknesses such as maintenance that may not be under your department's control, but these weaknesses should not be listed in #4 and #5 below.
