The CSU Institutional Review Board (IRB) utilizes several forms that are required for protocol review. You may download versions of each form using either of two formats: MS Word or a PDF file. The primary investigator shall bear final responsibility for submitting all forms required for review under IRB procedures. It is the responsibility of the investigator to use the most current version of each form needed. Please refer to this webpage for the most current form for the CSU IRB.
