Semiannual Certification of Eligibility for Dependent Healthcare Coverage - Response Due: March 17, 2010

CMS members must verify healthcare eligibility for dependent coverage by March 17, 2010.  If you have dependents who are covered under your health plan and are age 19 or over, you MUST complete an Eligibility Certification Statement, sign and return with any required documentation to the Office of Human Resources - ADM 203. Please email: hr@csu.edu or call 773.995.2040 with any questions. 

If eligibility certification statement is not received by due date, your dependant coverage will terminate effective 4/1/2010.

The Office of Human Resources will like to announce Wellness Wednesdays. Wellness Wednesdays is a program with the employee in mind. The programs that will be offered will focus on health, finance, emotional support and opportunities. The forum will be open for individuals to learn, listen and consider a host of perspectives and ideas. Wellness Wednesdays will be held on the third Wednesday of every month. If you have ideas and/or would like a topic to be discussed, please contact the Office of Human Resources with your suggestions.
