MANDATORY HEALTH INSURANCE COVERAGE
It is mandatory that full-time Chicago State University students have sickness and injury insurance. Students enrolled as follows are automatically charged each semester for the mandatory insurance plan provided through the university.
DOCTORATE: 6 Credit Hours or More
GRADUATE: 9 Credit Hours or More
UNDERGRADUATE: 12 Credit Hours or More
The mandatory requirement applies to students who enroll full-time in all on-campus courses or a combination of off-campus (Extension) and on-campus courses. Note that students enrolled in all web/online courses are not charged for the insurance plan.
Students must complete and submit an online waiver or enrollment form each Fall and Spring semester in which they are enrolled full-time.
WAIVING OUT OF/ENROLLING INTO THE PLAN EACH SEMESTER
IF YOU HAVE INSURANCE: Students who are adequately insured have the option of waiving out of the mandatory health plan and having the charge removed from their tuition bills. Failure to submit an online waiver or enrollment form by the deadline will obligate students to pay the FT Student Insurance Premium coverage indicated on your tuition bill. If your waiver is denied, you may provide proof of coverage to the Office of Student Health Insurance for reconsideration.
IF YOU ARE NOT INSURED: Students who do not have insurance must purchase the mandatory insurance provided through the university.
WHAT COVERAGE IS ACCEPTABLE: Adequate insurance must be provided through an acceptable health insurance underwriter such as Aetna, Blue Cross/Blue Shield, United Healthcare, Cigna, to name a few, and must be at least comparable in coverage to the plan provided through CSU. You may review the plan offered by the university under “Find My School Plan” at http://www.uhcsr.com (hold down “Ctrl” key to click onto the link).
Clinic cards for Cook County Hospital, Board of Health, and State of Illinois Healthy Womens’ Clinic are among some of the types of health coverage that are not acceptable. Health provisions must not be limited to specific facilities and must provide coverage anywhere in the United States.
IF YOU WANT TO PURCHASE INSURANCE: Students who wish to purchase the mandatory Student Injury and Sickness Insurance Plan provided through the university must complete and submit an online enrollment form to activate coverage as soon as possible. The period of coverage for Spring Semester 2012 is January 5, 2012 through August 18, 2012.
TO SETTLE DISPUTES: You will receive an email at your CSU email to confirm that your waiver or enrollment form was accepted or denied. Your confirmation email will be required for any waiver/enrollment disputes. PLEASE NOTE: CSU reserves the right to reverse waivers that were originally approved.
CHOICE PLAN
Each eligible student has a choice of one of the benefit plans. Plan 1 is a basic sickness and injury insurance plan without maternity coverage. Plan 2 has basic sickness and injury with maternity included in the benefit. Please be aware that if you choose to upgrade to Plan 2 in any subsequent policy year, a new Pre-Existing Condition exclusion and waiting period will apply. Students interested in the maternity option should contact the Office of Student Health Insurance.
NOTIFICATION TO GO ONLINE TO WAIVE/ENROLL
Before a student can go online to waive out of or enroll into the mandatory insurance plan, she/he must have been enrolled full-time for at least 24-48 hours. This window of time allows the Office of Student Health Insurance ample time to retrieve your registration status and upload it into the United Healthcare website.
As students’ names are uploaded into the insurance website, they will receive notification through their CSU email addresses letting them know that they are then able to log in to the website to complete a waiver/enrollment form.

The waiver/enrollment period for Spring Semester 2012 is as follows:

Ends: January 20, 2012 at Midnight
INSURANCE PLAN INFORMATION
The plan provided through Chicago State University is underwritten by United Healthcare Insurance Company and is based on policy 2011- 2363-1/2 for Fall Semester 2011. The plan brochure provides details of coverage including costs, benefits, exclusions, any reductions or limitations and the terms under which coverage may remain in force. PART-TIME STUDENTS WHO NEED INSURANCE
Part-time students who enroll in 6-11 credit hours of coursework will be allowed to enroll into the student health insurance plan provided through the university on a voluntary basis. However, you must contact the insurance provider directly, either online or by phone, to enroll. Click the following link to enroll voluntarily http://www.uhcsr.com.
COVERAGE FOR DEPENDENTS AND SPOUSES
Eligible students who do enroll may also insure their Dependents. Eligible Dependents are the spouse or Domestic Partner and unmarried children under 19 years of age, or 26 years of age if a full-time student attending any accredited institution of higher learning and is not self supporting.

CHECKLIST OF REMINDERS:
- THE ONLINE WAIVER/ ENROLLMENT PERIOD FOR SPRING SEMESTER 2012 ENDS AT MIDNIGHT ON JANUARY 20, 2012.
- FAILURE TO SUBMIT AN ONLINE WAIVER OR ENROLLMENT FORM BY THE DEADLINE WILL OBLIGATE STUDENTS TO PAY THE FT STUDENT INSURANCE COVERAGE AND ACCEPT COVERAGE.
- YOU MUST COMPLETE AND SUBMIT A WAIVER/ENROLLMENT FORM EACH FALL AND SPRING SEMESTER THAT YOU ARE ENROLLED FULL TIME.
- YOU MUST REGISTER INTO YOUR CLASSES AT LEAST 24-48 HOURS BEFORE YOU ARE ABLE TO GO ONLINE TO WAIVE OUT OF OR ENROLL INTO THE PLAN.
- TO BUY INSURANCE, YOU MUST SUBMIT AN ENROLLMENT FORM TO ACTIVATE COVERAGE AS SOON AS POSSIBLE.
- IF YOUR WAIVER IS DENIED, YOU MAY PROVIDE PROOF OF COVERAGE TO THE OFFICE OF STUDENT HEALTH INSURANCE FOR RECONSIDERATION.

- ALL FULL-TIME INTERNATIONAL STUDENTS ARE REQUIRED TO PURCHASE THIS INSURANCE PLAN FOR THE SCHOOL YEAR.
- PART-TIME STUDENTS TAKING 6-11 CREDIT HOURS MAY PARTICIPATE IN THIS PLAN ON A VOLUNTARY BASIS.
