Illinois Residency

 

The Illinois Student Assistance Commission (ISAC) requires that proof of Illinois residency be provided to schools in order to finalize eligibility for certain State of Illinois gift assistance programs such as the Monetary Award Program (MAP). In the instance of Dependent students, the requirement is that proof of Illinois residency be provided for the parent whose information is included on the student’s FAFSA.

If you have been notified by Chicago State University that such verification of Illinois residency is required please download the appropriate form from below, complete it and submit it to the Office of Student Financial Aid along with the required documentation attached evidencing Illinois residency.
