Download, complete and submit this form to our office as soon as you are notified that you have been selected for Verification. Remember to attach all required 2007 tax information. Don't forget - all tax returns (1040s and transcripts) must be signed.

Request a copy of a Federal Income Tax Form

You will, in most cases, need a copy of your 2007 Federal Income Tax Form and W2s, or a Tax Transcript and duplicate W2s, or a Statement of Non-Filing from the IRS (and your parents forms too if you are a dependent student) in order to complete the Verification process. If you can't locate these important documents you can easily obtain copies, free of charge, from the IRS.
