Download, complete and submit this form to our office as soon as you are notified that you have been selected for Verification. Remember to attach all required 2006 tax information. Don't forget - all tax returns (1040s and transcripts) must be signed.

Business Value Statement for Tax Year 2006 (.pdf)

If you submitted a U.S. 1040 to our office as part of the Verification process and that 1040 indicated that the filer had "Business income or loss" for 2006, the filer must also submit a Business Value Statement to our office.

Investment Value Statement for Tax Year 2006 (from Rental Real Estate) (.pdf)

If you submitted a U.S. 1040 as part of the Verification process and that 1040 indicated that the filer had "Income or Loss from Real Estate" during 2006, the filer must also submit an Investment Value Statement to our office.

If a Schedule E was filed relating to income or loss derived from an investment other than real estate  do not use this form. Instead, the filer should download, review and complete the form below.

Investment Value Statement for Tax Year 2006 (from other than Rental Real Estate) (.pdf)

If you submitted a U.S. 1040 as part of the Verification process and that 1040 indicated that the filer had "Income or Loss" from "royalties, partnerships, S corporations, trusts, etc." during 2006, the filer must also submit an Investment Value Statement to our office.

If a Schedule E was filed relating to income or loss derived from an investment in real estate do not use this form. Instead, the filer should download, review and complete the form above.

Request a copy of a Federal Income Tax Form

You will, in most cases, need a copy of your 2006 Federal Income Tax Form and W2s, or a Tax Transcript and duplicate W2s, or a Statement of Non-Filing from the IRS (and your parents forms too if you are a dependent student) in order to complete the Verification process. If you can't locate these important documents you can easily obtain copies, free of charge, from the IRS.
