Book Voucher Policy
Spring 2008
Book vouchers will be available to Chicago State University students who have:
- Completed registration
- Validated a payment method with the Cashier
- A credit balance for the current term
Vouchers will be issued based on the amount of the student’s credit balance up to $800.
Students with grants will be required to use it for their book vouchers before other forms of financial aid.
To obtain a book voucher, a student must present a current CSU identification card and follow the rules and procedures below:

*** Books and supplies will be issued in BHS 102 (only) all other items will remain in the old library Room 120***
1. Student’s name and social security number will be on a file in the bookstore. We will no longer issue paper vouchers. Students may make repeat purchases in the book store until the authorized amount is depleted.
2. Books and supplies purchased with book vouchers will be charged to the student’s account in the amount of the actual purchase. The student's signature constitutes authorization to apply the purchases to your account.
3. The Bookstore will issue refunds and make exchanges in accordance with its posted refund policy. Book Vouchers for the Spring 2008 Semester will be on file in the Chicago State University Book Store.

This is to acknowledge receipt of books issued under the CSU Book Voucher System. I understand and agree to abide by the rules of the CSU Book Voucher System, which includes the following:
1. Chicago State University or the CSU Bookstore are not responsible for lost
or stolen vouchers.
2. This voucher is valid for the Spring 2008 semester only. This voucher is void
After February 1, 2008.
3. Books and supplies purchased with the voucher will be charged to my account with the University.
I have read the rules of the CSU Book Voucher System Agreement, and I agree to abide by all conditions of this program.
