STUDENT HEALTH INSURANCE

MANDATORY HEALTH INSURANCE COVERAGE
Chicago State University provides a mandatory Injury and Sickness Insurance Plan underwritten by United Healthcare Insurance Company. All full-time students (undergraduates enrolled in 12 credit hours or more, graduates enrolled in 9 credit hours or more, and doctoral students enrolled in 6 credit hours or more) are automatically charged for the plan. All full-time international students are eligible for the plan and must be enrolled on a mandatory basis.
STUDENTS WHO HAVE HEALTH INSURANCE COVERAGEStudents who are currently covered by injury and sickness insurance will be able to waive out of the mandatory health insurance plan by going online during the specified waiver/enrollment period and completing the waiver form provided there. Waiver/Enrollment periods and deadlines are published in the Class Schedule Bulletin each semester. Students must complete an online form to waive out of or enroll into the insurance plan every fall and spring semester that they have enrolled full time according to their respective programs. Any student who does not submit an online waiver or enrollment form by the deadline will be required to buy the university insurance plan. PART-TIME STUDENTS WHO WANT THE INSURANCE PLAN
Part-time students enrolled in 6–11 credit hours of coursework may enroll in the student health insurance plan on a voluntary basis. Visit the United Healthcare website at www.uhcsr.com and click on the “Find My School’s Plan” link and follow the online instructions or call 800-767-0700. COVERAGE FOR DEPENDENTS AND SPOUSES Chicago State University students enrolled in the students health insurance plan provided through the university have the option of enrolling their eligible dependents and spouses into the plan for an additional cost. Visit the United Healthcare website at www.uhcsr.com and click on the “Find My School’s Plan” link and follow the online instructions or call 800-767-0700.
CONTACT INFORMATION United Healthcare Insurance Company is the current provider for Chicago State University full-time students who purchase insurance through the university, part-time students who voluntarily enroll in the plan online, and students’ spouses and dependents. The Chicago State University Policy Number is 2010-2363-1/2. Copies of the Plan brochure may be viewed and downloaded at www.uhcsr.com. The brochure provides details of the coverage, including costs, benefits, exclusions, any reductions or limitations, and the terms under which the coverage may remain in force.
