Mandatory Health Insurance Program
Chicago State University provides a mandatory Injury and Sickness
Insurance Plan underwritten by United Healthcare Insurance Company.
During the fall and spring semesters only, all full-time students
(undergraduates enrolled in 12 hours or more, graduates enrolled in 9
hours or more, and doctoral students enrolled in 6 hours or more) are
automatically charged for the plan. All full-time international students
must be enrolled into the plan on a mandatory basis. The fee is applied
to students enrolled in all on-campus courses or a combination of on-campus
and off-campus (Extension) or web courses.
Summer Waiver/Enrollment Period
Students generally do not enroll full time for the summer session and,
therefore, are not charged for the mandatory insurance plan. However,
anyone enrolled in at least six (6) credit hours who is interested in
obtaining the university insurance coverage during the summer session
only should contact the Office of Student health Insurance before July
19, 2011.
Part-Time Students Eligible for Coverage
Students enrolled in at least six (6) credit hours during the summer
session (enrollment in all web/online courses not accepted) may enroll
in the program on a voluntary basis. The voluntary enrollment deadline
is July 19, 2011.
Eligible dependents and domestic partners of enrolled students may
also participate in the plan on a voluntary basis. Inquire within the
Office of Student Health Insurance for details.
Period of Coverage
Students attending CSU for the summer session who did not attend
the previous spring semester may enroll for the CSU insurance plan if
they are enrolled for at least 6 credit hours. The period of coverage will
be May 22, 2011 through August 18, 2011. You may obtain a copy of
the 2010-2011 Student Injury and Sickness Insurance Plan brochure
by visiting www.uhcsr.com and clicking on the “Find My School’s Plan”
link and following the online instructions, or by visiting the Office of
Student Health Insurance.
Cancellation Requirements
Students generally do not enroll full time during the summer session
and, therefore, are not charged for the mandatory insurance plan.
However, if your account is charged for the insurance, contact the Office
of Student Health Insurance at 773-995-4533.
