Adjunct Faculty Appointment Process

In order to become a preceptor for CSU-COP, all potential preceptors must complete the adjunct faculty appointment process by submitting:

• The Preceptor Application for Faculty Appointment click here to download application
• Current curriculum vitae or resume;
• A copy of your current Illinois Department of Financial and Professional Regulation Pharmacist License (or the appropriate regulatory entity); and
• The Pharmacy Practice Experience Site Profile; and
• Complete the CSU-COP Preceptor Orientation Module http://www..csu.edu/pharmacy/experientialeducation/preceptored.htm

Upon receipt of the above, your application will be reviewed and the Office of Experiential Education will contact you. Upon approval, notification of the appointment will be sent to you via US mail. All adjunct appointments are contingent upon approval by both the College and the University, are effective from the date of approval through June 30th of the following year and are reviewed annually by the COP Department of Pharmacy Practice and Administration for retention.
