This form is for individuals who already have a bachelor's degree, are NOT certified teachers, and want to begin a teacher certification program. Complete this form and submit it with proof of your bachelor’s degree, transcripts, and a $40 check or money order to the certification counselor for the program you wish to apply. Focused Program Application for subsequent certification
This form is for individuals who already have a teacher certificate and want to earn a DIFFERENT TYPE of teacher certificate. Use this to earn a type 03, 04 09 or 10 in art, music, PE or Spanish. Complete this form and submit it with proof of your teacher certificate, transcripts, and a $40 check or money order to the certification couselor for the program you wish to apply.. Master’s Degree Application for an Advanced Certificate Program
Individuals who already hold an initial or standard teacher certificate (not a substitute or provisional vocational certificate) and want to pursue a master's degree program in Reading, Library Science, Special Education, or General Administration, or want to pursue a program in School Counseling or School Social Work, apply to the Office of Graduate and Professional Studies.
Certified Teacher At-Large Application
This form is for individuals who already hold an Illinois teacher certificate and want to take courses for an endorsement, but do not want to apply for a master’s degree or subsequent certificate program. Click here for application.
Formal Admission to the College of Education
The COE Application is for students currently enrolled in teacher education programs at CSU who are applying for admission to the professional phase of their programs. Complete the appropriate application packet below:
