Undergraduates
   
    Obtain Blue Application Packet from Department of COE
   
    Note the testing schedule for the Basic Skills Test (ICTBS)

    Be sure that department requirements have been completed  
          Obtain Department Evaluation

    Advisor Certifies that
          all university exams have been passed

          you have registered for the ICTBS

          you are within 12 hours of completing your general education requirements

          you are listed under the correct major code (PHYT)

   Advisor attaches updated copy of evaluation form

    Submit department recommendation request
          department keeps a copy
          mail original to the OTC

    Department returns completed application to candidate - candidate submits to OTC, ED 208 with passing ICTBS score.

Post baccalaureate Students

    Applications in OTC

    Non-degree Certification Only

    Masters Degree with Initial Certification (MAT)
