Procedures for students requesting services
All services require at least a two-week notification. ( In some circumstances services may be provided sooner but ample notification is still required.) Services and accommodations do not include the following: personal attendants, individually prescribed devices, or readers/typists for personal use or study time.
Telephone Hotline
The Abilities Office recently established a telephone hotline which can be accessed through our general office telephone number (773/995-4401). This hotline, which was initiated in an effort to assure that all students have access to campus announcements, provides verbal details of CSU events and announcements. To hear these announcements, call the Abilities Office at 773/995-4401 and then press the number “1” when prompted. Any questions regarding this system should be directed to the Abilities Office at the number listed above.

Typing Services (when appropriate) |
|
|
When submitting handwritten papers to
be typed, please make sure that you have indicated the format style
that you desire. (i.e. double space, single space, etc. ) If there
are no detailed specifications the paper will be typed in the given
form.

|
|
Handwriting must be legible.
|
|
|
Make sure that all spelling and grammar
are correct. Please note that it is not the service provider's job
to make spelling and grammar corrections. |
|
|
The assignment must be submitted to the Abilities
Office of Disabled Student Services in ample time for one revision
before it is returned to the professor as a completed assignment.
All of the typists providing the services are students. Therefore,
it is crucial that you take advantage of the one opportunity to
proof and make revisions.All assignments will indicate that the
typing services were provided by the Abilities Office of Disabled
Student Services.

Tape Transcription |

recorded information on a cassette for the purpose of transforming
to typewritten form) please indicate the following: |

Equipment Loan |
|
No equipment will be loaned out if a student's
file is not complete.(Application on file, medical documentation
and current class schedule.) The appropriate form must be completed
in order to receive equipment.

Note takers |
|
A note taker may be provided for you . The service
provider may also be a student in the same class. We will provide
copy set paper so that a student can take notes and share a copy
on the day they are taken.

SCRIBE PROCEDURES |
|
|
A scribe will write down, verbatim, what the student has dictated. A student at any time will have the opportunity to review what the scribe has written either by reading or having
it read to him/her. If there are corrections, the student will direct the scribe to make them.
|
|
|
A scribe will be responsible for spelling and sentence ending punctuation. Student must direct the scribe for any specific spelling or punctuation within the sentence.
|
|
|
A scribe is not responsible for organizing nor paraphrasing student's thoughts into final form.
|
|
|
A scribe may stop the session at anytime and get Abilities Office staff if he/she feels that the student is requesting more assistance than is appropriate.
|
|
|
Abilities staff will review the above procedures with the student and scribe at the beginning of each session.
