INTERPRETER/ NOTE TAKER SERVICES AGREEMENT |

When requesting interpreter and note taker services through the Abilities Office, it is important that you understand and follow these office procedures and rules:

How To Request Services: Set up an appointment with the coordinator of Disabled Student Services and bring with you current documentation of your disability. Fill out and turn in your service request form as early as possible each semester. Students requesting interpreters for the following semester must register and turn in their Services Request form eight (8) weeks before classes begins. During the semester, if you need to request additional interpreter and note taker services outside of class time (for special events on campus, appointments with advisors, etc.) you must fill out another Service Request form with the exact dates the services are needed. We require 48 hours notice for note takers and two weeks notice for interpreters outside of class time.
Interpreter's Job Duties: The interpreter sits near the instructor, facing the student, and interprets all spoken and signed communication in the classroom. The interpreter cannot give opinions or advice, and cannot chat with you during class time. The interpreter also cannot answer your questions about the class, but will interpret for you while you ask your teacher questions. If you have any concerns about your interpreter services, please inform the coordinator of Disabled Students Services right away.

Note taker's Job Duties: The note taker sits next to you in class and writes down information given in the lecture and written on the board. The notes should be complete and clear. If you have any questions about the notes, talk with your note taker after class. If you have concerns about the quality of the notes you receive, please inform the coordinator of Disabled Student Services right away.

What To Do if Interpreter or Note Taker is Absent : If an interpreter or note taker has not yet been assigned to your class, or is absent, we will try to assign a substitute. However, if you arrive at your class, and your interpreter or note taker is absent, please be prepared to do the following:

Tape record the class and bring the tape to the Abilities Office right away and we will transcribe notes for you from the tape. You will need to have an audio cassette to put in the tape recorder. Bring a cassette tape to school every day in case you need it. Return borrowed tapes to us at the end of the semester.

What Must You Do If You Will Be Absent: A student who has requested interpreter services and then finds that he or she will not be attending the class or event in question, must cancel the arrangements by notifying the Abilities 0ffice at least two (2) business days before the class or event is to take place. Call the Abilities Office at (773)995-4401.

If a student has not arrived by fifteen (15) minutes after a class session or event is scheduled to begin, the interpreter and note taker will leave and the student will be considered ''absent.'' (For classes that are two hours or longer, the grace time is 30 minutes.)

Suspension of Services:If a student has more than three (3) absences in a given course, he or she will be notified that interpreting services for that course will be withdrawn for the remainder of the semester. If, during the course of a semester, a student has more than five (5) total absences from classes or other events for which interpreter services have been requested and provided, the student will be notified that all interpreter services will be withdrawn for that student for the remainder of the semester.

Reinstatement of Services: If a student's services have been suspended, the student may reapply for interpretive services by filling out a new Service Request Form. The student must also schedule an appointment to meet with the Abilities Office coordinator to discuss accommodation issues. Please note, reinstatement of services may not be immediate and may take two (2) weeks for an interpreter and note taker to be assigned.

Dropping A Class: If you plan to drop a class, inform the coordinator of Disabled Student Services immediately so interpreter and note taker services for that class can be canceled.

EXAM PROCTORING POLICY
The Abilities Office is responsible for proctoring exams per the faculty member's instructions. These instructions are documented and kept on file. The instructions are reviewed with the student before taking the exam. The student is responsible for following those instructions at all times. A staff member may enter the exam room at any time to monitor the exam conditions. Any suspected evidence of cheating will be documented and reported immediately to the faculty member. As a result of this the student may be charged with academic misconduct.
