- Students (with 12 hours or more) are automatically
billed and enrolled in the mandatory health plan with the option
of canceling each semester.
- Students with equal or better coverage may
apply for cancellation of the mandatory fee prior to the deadline
date, which is printed in the Class Schedule Bulletin each semester.
The basic Student Health Plan for accident and
sickness is mandatory for students, undergraduate and graduate,
who enroll with 12 hours or more. The fee applies to all on-campus
semester hours and students with a combination of off-campus semester
hours and on-campus semester hours. Students are automatically
charged a fee for student health insurance on their tuition account
through the Bursar’s Office each time the enrolled hours
become 12 or more.
APPLICATION FOR CANCELLATION AND CANCELLATION
REQUIREMENTS
A cancellation form must be submitted every semester
a student enrolls with at least 12 credit hours, by the deadline
date, or the student will be held responsible for the mandatory
Student Health Insurance Fee.
- Copy of insurance card and ID must accompany
cancellation form.
Students eligible to cancel should receive a
credit to their account within 30 days of the cancellation deadline
date.
OTHER STUDENTS ELIGIBLE FOR PARTICIPATION
IN THE PLAN
Insured students have the option of enrolling
their spouses and any unmarried dependent children up to 19 years
of age or up to age 23 if your dependent is a full-time college
student.
Part-time students (with 6-11 hours) will be
allowed to enroll in the Student Health Insurance Plan on a voluntary
enrollment basis.
