MANDATORY HEALTH INSURANCE COVERAGE

 

It is mandatory that Chicago State University students, graduates and undergraduates, who enroll in 12 credit hours or more must carry health insurance that will cover accidents and sickness.  This requirement applies to students who enroll in all on-campus courses or a combination of off-campus (Extension) and on-campus courses.  Students are automatically charged a health insurance fee on their tuition accounts and enrolled in the health plan provided through the university.  However, those who are eligible have the option of cancelling enrollment in the mandatory health plan and receiving a refund.

 

STUDENTS COVERED UNDER THE UNIVERSITY PLAN

 

Students who are required to remain on the student health insurance plan provided through the university need do nothing.  Once the deadline for cancelling the university health plan is past, the university will submit the names of all who are to be insured through the plan to the insurance company, who in turn will mail your insurance ID card to you. 

 

STUDENTS WHO HAVE HEALTH INSURANCE COVERAGE

 

Students who possess health insurance may cancel the health plan provided through the university.  Note that every semester that a student enrolls in 12 or more credit hours of courses, they are automatically charged a fee for the health insurance plan provided through the university, and must submit a request to cancel the plan and fee.  The deadline by which one must cancel the health insurance plan and fee is published online each semester in the Class Schedule Bulletin.  Those eligible to cancel the insurance plan available through the university must provide the following:

 

  • An insurance card showing your full (first and last) name, or a letter from the insurance company naming you as insured.
  • A CSU student ID displaying a sticker for the current semester.

 

Cancellation forms must be submitted every semester that a student enrolls in 12 or more credit hours of coursework.  The forms must be completed in the Office of Student Health Insurance.

 

PART-TIME STUDENTS WHO NEED INSURANCE

 

Part-time students who enroll in 6-11 credit hours of coursework will be allowed to enroll in the student health insurance plan provided through the university on a voluntary basis.  However, you must contact the insurance provider directly, either online or by phone, to enroll. 

 

COVERAGE FOR DEPENDENTS AND SPOUSES

 

Chicago State University students enrolled in the student health insurance plan provided through the university have the option of enrolling their eligible dependents and spouses into the plan for an additional cost.
