Application Process
Step 1:
Download and complete the Graduate School Application (i.e., Graduate Admission Form). The processing fee is $25.00, which should accompany your Graduate Application. Also, consult the CSU Graduate catalog before applying. The Graduate School will process your application after all materials have been submitted. Our advice is to do the following: First, gather ALL necessary information required for the Graduate Application - official transcripts, letters of recommendation, etc. Then, mail everything at once to the Graduate School:
Chicago State University
School of Graduate and Professional Studies
9501 So. King Drive
NLB Rm 234
Chicago, IL 60628-1598
Tel: 773. 995. 2404
Fax: 773. 995. 3671
Email: G-Studies1@csu.edu
Step 2:
Once you meet the graduate school requirements, your application will be forwarded to the HPER department. At the department level, the HPER Graduate Program Director will determine whether the student has satisfied the prerequisite requirements. If all prerequisites have been completed (as shown on official transcripts), conditional or full admission is determined based on the student's GPA. If all prerequisites have not been met, a letter to that effect is sent to the student; otherwise, a letter of admission (conditional or full admit) is sent out. This letter contains VERY IMPORTANT information for which the student is responsible.
Step 3:
You take courses and record this information on your Graduate Advising and Program Planning (GAPP) form - see the Forms and Paperwork and Registration links on the HPER PE Graduate web page. Please Note: You MUST BE FORMALLY ADMITTED TO THE UNIVERSITY before registering. If you’re unsure of your status contact the Office of Graduate Studies at (773) 995-2404.
