Since I'm not going to the graduation ceremony will my diploma be mailed to me, or what do I need to do?
A.
Usually if you don't go to the graduation ceremony the graduate school will send your diploma to you.

I received a letter about a class offered through CSU at Joliet Junior College from July 9-13th. It says you will get 3 hours of graduate credit for through Chicago State. Does this apply to our grad program? I have taken PE 495 for 1 hour of grad credit before. Can you please let me know if this is acceptable credit toward the grad program?
A.
The Joliet Junior College Cooper Institute Certification can count as a 3-credit elective. Similar to the IAHPERD 1-credit elective, this course is offered as PE 495 and PE 499. Click here for a PDF of the 2008 brochure for the Certification Workshop. NOTE: The cost of the workshop is $600.00 and the cost for the 3 Chicago State credits is $603.00. So, if you simply do the workshop your cost is 600.00. If you do the workshop and want it to count as 3 elective credits toward your PE masters at CSU, your cost is $1203.00. Also, you should keep in mind that if you have taken the IAHPERD course you should register for the Joliet Course using a course number that is different than that of the IAHPERD course because some districts won't accept two courses with the same number.

I registered for a course for fall semester, but will be unable to take it do to schedule conflicts. I did not pay for the course yet, but I am registered through the web. The registrar's office told me I need to come in person to withdraw from the class by Tuesday in order to not get stuck paying for it. I unfortunately will never make it to the city from my house, because I coach after school too. Could you please fill out the paperwork and bring it to the registrar’s office by Tuesday?
A.
No. Once you register for a course, it is your responsibility to pay for it or drop it. Students who need to drop or withdraw from a course for any circumstance that requires a Drop form (e.g., when students cannot drop via CSU X-press because they missed the drop date deadline), must make arrangements with their advisor, travel to Chicago State, pay for parking (~ $4.00), fill out the Drop form, obtain an advisor signature, and take the form to the Registrar (ADM bldg rm 128) to drop the course. HPER department faculty will not do this transaction for a student, nor will they do it via fax or email – there are no exceptions to this policy.

I was wondering if you could look over my transcripts and let me know what I need to do to graduate this semester. I know that I need 424, 425, and 426. I believe that I need 4 more credits to be done after that.
A.
You go to CSU X-Press and obtain your online transcript, then you compare that with the Program Planning form or "GAPP" form on the HPER graduate web page.

If possible, I need a letter stating that I have fulfilled all requirements to obtain a masters degree. My principal said that the letter along with unofficial transcripts would suffice for a lane change. Thank You.
A.
Faculty will not write letters to school district supervisors stating that a student has the master's degree. You must apply for graduation and follow the process. We cannot accelerate the date or the process simply because a student failed to plan in advance. Both the Graduate School and faculty agree that faculty should not write letters to school districts stating that a teacher in their district has the degree until their materials have been audited according to process and policy.

Do you offer 1-credit hour independent study courses at the graduate level?
A.
No. However, we do offer a 1-credit elective as part of the IAHPERD conference held every November in St. Charles, IL.

I think there may be a hold on my account. CSU Xpress says: Immunization Hold Oct 24, 2008 to Dec 31, 2099. Do I have to do something about this?
A.
Yes. The Illinois Department of Public Health specifies that any student registered for 6 hours or more and born on or after January 1, 1957 who is either a first time freshman, a transfer student, or a student reentering the university after an absence of more than one semester will be required to show proof of immunity to: Tetanus diptheria, measles, mumps, rubella (MMR) - documentation of 2 live doses of measles, mumps and rubella virus vaccine or proof of immunity by titer. Tuberculin skin test (PPD/Mantoux test) is required for residence hall students and some academic majors. Students who wish to document that they have met the immunization requirements must complete the immunization form and send it to: Wellness/Health Center, Chicago State University, 9501 King Drive, ADM 131, Chicago IL 60628. I know I’m required to complete 10 hours of electives. Do those 10 hours all have to be in the same area or can I take some in health education and some in adventure education and use both subjects to get the hours needed to complete 10?
A.
Elective courses can be from other related areas as long as they are graduate level. One thing you can't do is double dip, that is, use a class from the main required course list (or the prerequisites) as a course for the electives too. The best way to get that 1 credit elective is to take the 1 credit IAHPERD course in the fall at the IAHPERD Convention in St. Charles. I need an e-mail account. How can I get an e-mail account?
A.
After you are admitted to the program, you should receive a CSU email account. Contact the Information Technology Help Desk 773-995-3963 to obtain your username and password. It is important that you obtain and activate your CSU email account as Blackboard uses your CSU email for messages and announcements.
We recommend you have your CSU email messages forwarded to another account. To do so, follow these steps:
1. Go to the CSU homepage and click on eMail
2. Enter your user ID and password. Your user ID is the part of your email address that is before the @ sign.
3. Once you've entered the email click on the button at the top labeled Pref (i.e., "preferences")
4. In "Forward mail to", enter the email address you want your CSU email forwarded to
5. Also be sure the box to the right labeled "Keep local copy" is unchecked
6. Scroll down to the bottom of the page and click "Save".

Prerequisites
A.
Our program is committed to producing graduates who are well rounded and well versed in the field of physical education. To this end, we require completion of undergraduate courses or their equivalents in kinesiology (or "biomechanics"), measurement and evaluation, exercise physiology and adapted physical education, as prerequisites for the program. These prerequisite courses must be completed before applying to the program.
We understand that working professionals, who are interested in our program but do not have a degree in physical education, have little time during the day to complete these courses. There are however, several universities that offer these courses online. Please investigate the following links as a viable choice for completing these prerequisities.

Once at the site, click on class schedules, then select semester from the pulldown menu and click Submit. For Subject, try Exercise Science then scroll down and click on Class Search.
