Service Request

Please complete and submit an online service request form for issues pertaining to pc repair or servicing and classroom equipment request.  The service request form is available online in pdf form. 

You can type directly into the pdf form and submit the form by clicking on the submit button located in the lower right corner of the screen.  Once you press submit, you will be prompted to select send mail option. Just press enter, then an email screen will appear.  Once in e-mail, select send.  Your request will go to cite@csu.edu.
